Important notices from Human Resources

In accordance with federal and state regulations, the University of South Alabama is required to annually post certain information and notifications to employees.

Annual Notice of Federal Laws and Acts – USA Choice Plan & USA Select Plan – Benefit Year 2023 - This notice provides important information about federal laws and acts that affect your coverage with the Plans. Along with your Summary Plan Description, this notice will assist you in understanding the Plan’s policies and procedures; and your responsibilities to the Plan.

Medicare Part D Notice - 2023 - CMS regulations require that employer-sponsored prescription drug plans disclose credibility to all plan members annually. This notice specifically applies to plan members who are Medicare eligible.

Medicaid and the Children’s Health Insurance Program (CHIP) - Some states have premium assistance programs that can help employees who are eligible for coverage with the USA Choice & Select Plans but cannot afford to pay the premiums.

USA Drug & Alcohol Abuse Prevention Program - Statement of Policy - Title I of the Higher Education Act of 1965, as amended, requires that the University provide to employees an annual notice of the Drug & Alcohol Abuse Prevention Program Statement of Policy.

Reporting of Child Abuse and Neglect Policy - In compliance with Alabama law, with limited exceptions, employees who know or suspect a child (a person under the age of 18 years) is a victim of child abuse or neglect are required to make a report.

Notice of Title IX Policy - Title IX of the Education Amendments of 1972 is a federal law that prohibits sex discrimination in education.

Invitation to Self-Identify for Individuals with Disabilities and/or Protected Veterans - The University of South Alabama is a federal contractor subject to the following:

  • Section 503 of the Rehabilitation Act of 1973, as amended, requires government contractors to take affirmative action to employ and advance in employment qualified individuals with disabilities
  • Section 402 of the Vietnam Era Veterans’ Readjustment Assistance Act as amended by the Jobs for Veterans Act of 2002, requires government contractors to take affirmative action to employ and advance in employment qualified disabled veterans, recently separated veterans and other protected veterans.

Ethics and Compliance Hotline - The Ethics and Compliance Hotline is a tool where a member of the University community can, in good faith, report any suspected offenses of fraud, theft, waste, abuse, behavioral misconduct, or violations of University policies and/or state or federal laws.

Section 1557 - Notice of Nondiscrimination - Section 1557 is the nondiscrimination provision of the Affordable Care Act (ACA) which prohibits discrimination on the basis of race, color, national origin, sex (including pregnancy, sexual orientation, gender identity, and expression), religion, age, genetic information, disability or protected veteran status in certain health programs or activities.
