The process will create a more efficient and timely system for providers and ensure that follow-up appointments are scheduled.

USA Health has begun a new process to optimize the time it takes for patients to be discharged from the hospital.

Providers now can request a patient discharge using a centralized priority hospital line. The priority provider discharge request line will be answered during regular business hours, 8 a.m. to 5 p.m. Monday through Friday, by a member of USA Health’s Centralized Referral Team.

To request a hospital discharge appointment, call 251-910-1183.

“This new process will not only assist in creating a more efficient and timely process for our physicians, residents, nurses and ward clerks, but it also will ensure that patients have their follow-up appointments scheduled prior to discharge,” said Shelby Smith, director of care access for USA Health.

Providers who would like to request a discharge after hours should send a Cerner message to the Care Access Hospital Discharge Request pool. An appointment will be made at 10 a.m. the following day. Please use the Hospital Discharge Clinic Follow-Up Appointment Request template in the message center.
