We want to help you in requesting an appointment with our providers at USA Health. Use the form below to fill in your personal information, medical need, health insurance, and preferred doctor, if applicable.

Existing patients who have registered for the MyUSAHealth patient portal can use the portal to view or cancel previously scheduled appointments.

Please note that some of our specialists, such as High-Risk OB providers, require referrals from other providers, and new patients cannot schedule initial appointments themselves. To request an appointment with a High-Risk OB provider, please have your regular OB/GYN call the Center for Women’s Health at 251-415-1496 or fax their referral to 251-445-2462.
